FOIL Requests

For your convenience, we have a FOIL form listed below for you to print and complete.  If you prefer, you may send a letter or an email outlining the details of your request to the City Clerk/Deputy City Clerk.  Be sure to include the requested information such as requested name, date of birth (if known), date range (if applicable), address, and department from which records are requested (ie. Police, Fire, Codes, etc.). The reason we ask for the date of birth is there are times when some individuals have the same name, and this is a way to distinguish which person's records are needed.
