Cross-Connection Control & Backflow Prevention

The Cross-Connection Control Program helps protect and maintain the quality of the City’s drinking water supply. Backflow preventers are a critical component of the City of Oneida Water Department’s Cross-Connection Control Program.

Below are links to the Backflow Preventer (BFP) application documents that must be submitted to the City of Oneida Water Department. If you are unsure whether these documents are required for your project, please contact the Water Department at 315-363-1490.

Note: The City of Oneida Water Department will not begin construction of your water service until approval of the backflow prevention plan is received from the Department of Health. Please allow 4 to 6 weeks for the review process.

Backflow Preventer (BFP) Design Approval Process

The design approval process consists of three steps:

Step 1 – Initial Review

Submit your application documents electronically in PDF format to water@oneidacityny.gov. Please include the property address in the subject line.

Use the City of Oneida checklist to ensure all required materials are included. At this stage, owner and design professional signatures and stamps are not required.

The Water Superintendent will review the submission and contact you with any questions, comments, or required revisions.

Your design professional must follow the NYSDOH Guidelines for Cross-Connection Control, including all applicable drawing requirements.

Required Submittals:

  • City of Oneida Water Department Checklist
  • DOH Form 347 – Application for Approval of Backflow Prevention Devices
  • Site Plan and Location Plan
  • Plumbing Floor Plan
  • BFP Elevation and Plan View Details
  • City of Oneida Engineering Report Form (include supplemental narrative if needed)
  • Commercial Property Service and Meter Sizing Sheet (if applicable)
  • Manufacturer’s data sheets for BFP device(s), including FCCCHR of USC listing
  • Manufacturer’s data sheets for enclosure (“hot box”), if applicable

Step 2 – Final Review

Once the initial submission is deemed approvable, you will be asked to provide four (4) hard copies of the revised submittal package.

Final submissions must include all required:

  • Signatures
  • Dates
  • Professional Engineer (P.E.) or Registered Architect (R.A.) stamps

After receipt, the City will:

  1. Complete final review
  2. Sign the DOH Form 347
  3. Forward all four (4) copies to the County Health Department

The County Health Department will conduct their review and either:

  • Contact you with comments, or
  • Issue an approved, DOH-stamped set for your records

Step 3 – Certification of Installation

After installation, the DOH Form 1013 (Initial and Annual Test Report) must be completed.

  • The design professional must complete Parts A & B to certify the installation complies with approved plans
  • The completed form must be submitted within 45 days of installation to:
    • The City of Oneida Water Department
    • The County Department of Health

Annual Requirement:
Backflow preventers must be tested annually by a NYSDOH-certified tester. Submit test reports to both the City and County. For annual testing, only Part A of DOH Form 1013 is required.
